At Queensland Country Health Fund, we believe good health isn’t just about treating symptoms it’s about spotting risks early and giving our Members the right support before small issues become bigger ones.
That’s why our Care Navigation team has partnered with Queensland Country Dental to help identify Members who may benefit from extra health support.
Did you know your regular dental visit can reveal more than just the state of your teeth? Dentists are often the first to notice signs of broader health conditions. Recognising this, our Care Navigation Manager, Tina Clark, teamed up with Principal Dentist, Brendan Mudge, to create a simple referral process. Now, if the dental team notices indicators of chronic disease, they can connect Members directly with our Care Navigation Care Coordinators.
From there, our Care Coordinators reach out to Members, explain how the Chronic Disease Management Program works, and help guide them into the program with confidence and ease.
Since the referral process launched on 1 July 2025, the results have been encouraging:
• 74 referrals received so far.
• 7 Members already enrolled in the program.
• A further 29% of contacted Members have expressed interest.
This partnership shows how teamwork makes a real difference when dental professionals and Care Navigation work together, Members are connected to the right support at the right time.
Our Care Coordinators play an important role as the bridge between early detection and meaningful action. By guiding Members step by step, they make health management more personal and less overwhelming. Combined with the keen eye of our Queensland Country Dental team, this initiative helps Members stay on top of their overall wellbeing, not just their oral health.
